Summary
JOB SUMMARY: To work as part of the Paediatric Dietitians team in a variety of community settings including clinics, schools and nurseries and families homes as well as at the hospital, on the childrens ward and occasionally the neonatal unit. To undertake work that is delegated and also supervised by Paediatric Dietitians in order to enhance service delivery. SECTION 1 MAIN RESPONSIBILITIES: PROFESSIONAL/CLINICAL To support the Paediatric Dietitians, who work with patients in the community and acute settings, including children with complex medical needs. Some of this work is carried out alongside the ISCAN (Integrated Service for Children with Additional Needs) team. To support families in carrying out dietetic interventions as assessed for and planned by the dietitian. Be able to work independently under the supervision of a responsible practitioner and be responsible for monitoring/assessing the childs progress, modifying treatment and reporting back to the dietitian as appropriate. This will be in the child's home, school, playgroup/nursery, on the ward or other similar environment in Tameside and Glossop. To work with children and families on a one-to-one basis, in a group session or over the telephone. Some of the children may have complex medical needs, some may also have terminal illness or chronic degenerative conditions, and some will have poor family/social support. Some children and their families may be withdrawn or aggressive. To prepare equipment (such as scales, height measure) and resources as required ensuring treatment sessions are age and developmentally appropriate, child centred and fun. Be responsible for reporting back findings and progress from work with children and families to the dietitian, in order to inform planning and evaluation of care and management of risk. To provide concise relevant information, both written and verbal, about children's progress to dietitians and other practitioners/therapists, contribute to assessment and review reports as appropriate. Answer the telephone, take messages and deal with enquiries on behalf of the Paediatric Dietitians team taking the appropriate action Able to promote the development of the child/young person, through modelling and promoting reflective functioning and sensitivity to parents, in all settings. Give advice to families on local voluntary and statutory agencies who may be a source of extra support. Be responsible for making referrals to other agencies in response to identified needs, where agreed with the dietitian. Deliver/issue/order and collect equipment and feed e.g. for tube fed children requiring feed deliveries at home or the childrens ward requiring a stock of feed products. The post-holder will be dealing with children in a variety of environments and will become a significant contact to the children, their family and other involved individuals, but will be expected to maintain a professional relationship at all times. ADMINISTRATIVE To have keyboard skills as a computer will be used on a daily basis for data inputting. To maintain accurate, contemporaneous records in both patient records and electronic systems in relation to actions undertaken and advice given in accordance with Trust and departmental policies.
