Summary
The post holder is responsible for the delivery of practice nursing care to the practice population. Supported by the MDT practice team and the wider PCN team, they will deliver care within the boundaries of their role. Focusing on supporting patients to live healthy lives, monitoring of long-term conditions especially those with Diabetes, health prevention and screening activities. They will work collaboratively to meet the needs of patients, supporting the delivery of policy and procedures, as well as various primary care contracts such as the PCN DES, QOF and local improvement schemes.
