Summary
Support the repeat prescribing process, ensuring prescriptions are issued accurately and in line with clinical guidance. Conduct medication reconciliation after hospital discharge or clinic appointments, liaising with clinical staff as needed. Assist in the implementation and monitoring of medicines safety protocols (e.g. high-risk drugs, controlled drugs). Manage and maintain accurate patient medication records. Process prescription queries from patients, community pharmacies, and other stakeholders. Monitor and update drug monitoring alerts, working closely with clinicians and clinical pharmacist to ensure patient safety. Assist in identifying and managing patients suitable for repeat dispensing. Support the implementation of prescribing audits and quality improvement initiatives. Work with the practice team to reduce medication waste and promote cost-effective prescribing. Help coordinate and follow up on medication reviews, including sending invitations and updating records. Communicate relevant medicines-related information and changes to practice staff and patients. Liaise with community pharmacies, PCN and ICS pharmacy teams to ensure continuity of care. Participate in clinical and operational meetings, contributing to medicines optimisation planning. Support the delivery of national and local prescribing targets, such as QOF and IIF. Maintain up-to-date knowledge through continued professional development in line with GPhC requirements.
