Summary
This is an amazing opportunity to join our Patient Services team at Sandy Health Centre. Working collaboratively with the care navigation team, using your brilliant customer services skills, you will ensure patients are supported to access the care that they need. You will bring your skills and experience to life by: Deliver patient centred support to patients to access the care they need. Ensuring appointments are triaged effectively through digitised systems. Provide administrative support to the practice and clinicians to ensure patients care is processed correctly and efficiently. Where appropriate, signposting patients to alternative care and/or provide self-management advice and support.
