Summary
Act as an Important link between GPs, other Clinicians, external agencies and patients. Ensure seamless, patient care by collaborating with hospital teams, social services and community providers. Patient Centred care : Treat every patient with respect, kindness and confidentiality. Guide patients to the most appropriate services, or support within Primary Care Network (PCN)
