Summary
Independently assess, diagnose, and manage patients presenting with acute and chronic conditions Prescribe medications and treatments in line with clinical guidelines Order, interpret, and act on investigations such as blood tests, X-rays, and ECGs Develop and implement personalised care plans for patients with long-term conditions Provide health promotion and patient education to encourage self-management and wellbeing Make appropriate referrals to secondary care or other health and social care services when needed Work collaboratively with GPs, nurses, pharmacists, and other team members to deliver integrated care Participate in clinical audits, quality improvement projects, and clinical governance activities Maintain accurate and confidential patient records in compliance with legal and practice standards Contribute to the development and improvement of practice protocols and service Attend and actively participate in team meetings and professional development sessions
