Summary
Exciting opportunities have arisen following a review of provision of mental health services for older people at West London NHS Trust. We are in the fortunate position of transforming and expanding our service to meet the needs of older people with mental health needs.
We seek an experienced, committed, and enthusiastic nurse to join us as a band 6, working in the Specialist Older Adult Mental Health (SOAMHS) community team. The team provides community-based treatment and care to people over the age of 70 who have functional mental ill health and who are in need of support due to the complex nature of their presentation. The role of Community Nurse is to work in partnership with patients and their families/carers in OPMHS, developing their personalised care plans to enable them to lead fulfilling lives
The post holder will work under the supervision of a B7 Senior Practitioner. The post holder will carry a defined caseload of patients. They will utilise their clinical knowledge and skills in the assessment and care of Older Adults with functional mental health problems in the community. They will work within a multi-disciplinary team to undertake the facilitation of groups and individual therapeutic activities.
The post holder will be responsible for the supervision of junior staff and students as well as contributing to departmental and Trust improvement projects.
