Summary

We are looking for highly motivated, qualified social workers, who share our values of Enablement, Innovation and Respect, to join our expanding Adult Social Care Service, which was recently rated andlsquo;Goodandrsquo; by the Care Quality Commission.

We have a number of permanent posts available across our Short Term Service, Front Door Service, Long Term Service, Hospital Interface Team and our Deprivation of Liberty Safeguards Team.



Applications are invited from both experienced and newly qualified social workers.



The duties of the post include:

- Undertaking Strengths-Based assessments; developing and reviewing care and support plans to ensure they meet shared / best outcomes.

- Multi agency working in close partnership with customers, Carers and other stakeholders.

-Undertaking Assessments under the Mental Capacity Act



The successful candidates can expect to be well-supported in their professional development and will be provided with ample opportunities to ensure they meet the registration requirements of the Regulator as well as explore career progression pathways. We have a well established Assessed and Supported Year in Employment (ASYE) Framework and a clear Workforce Development Strategy in place. In turn, the successful candidates will have:



- Excellent communication and assessment skills, plus a solid understanding of the relevant statutory frameworks.

andnbsp;- The ability to evidence and demonstrate best practice as well as be accountable for their work.

andnbsp;- The ability to work with an appropriate level of autonomy, as well as within a busy social work team.

andnbsp;- The ability to meet targets and deadlines, including data quality.

andnbsp;- A Social Work qualification and be registered with SW England.andnbsp;

andnbsp;- The ability to work a flexible working pattern if required to meet changing demands.



The following are desirable:

- Working knowledge of the Care Act, Mental Health Act, Mental Capacity Act and Deprivation of Liberty Safeguards. andnbsp;

- Understanding of the Transforming Care Agenda.andnbsp;
