Summary

Assess acutely ill or deteriorating patients based on wards or appropriate departments and assist with their management plan and appropriate level of care, either avoiding admission to critical care or assisting with escalation to a higher level of care if necessary, in conjunction with the medical teams Ensure timely referral and admission to the appropriate critical care area for those patients who require a higher level of monitoring or clinical support than can be provided at general ward level Share knowledge and skills with clinical staff, both at the bedside and through formal education programmes within the Trust. With members of the multi-professional teams, respond to requests for support in assessing and managing the deteriorating and/or sicker ward patients. Manage caseload Outreach patients with the aim of avoiding admission to critical care, however facilitating timely admission where necessary. Following-up patients recently transferred out of Critical Care. To provide expert advice and education to clinical areas and all staff involved with patient care of this group of patients.
