Summary

To support patients/service users in undertaking activities of daily living within careplans 2. To contribute to the assessment and development of care plans under the directionof a Registered Health or Social Care Professional. 3. To undertake the role of health promoter and educator for patients/service users andtheir support networks. 4. To recognise changes in patients'/service users' presentation which require theintervention of others and understand when to refer to the supervising Registered Health or Social Care professional and other colleagues for reassessment of patientcare. 5. To report all accidents and incidents as per policy and demonstrate an awareness ofrisk management and Quality and Safety processes. 6. To report and escalate any safety, safeguarding or risk concerns to the appropriatemember of staff and ensure that action is taken. Communication 1. To build a therapeutic relationship with patients/service users and their supportnetwork, communicating with them in a way that respects their views. 2. To use a wide range of communication skills that can be adapted to meet the needsof complex and challenging situations. 3. To demonstrate effective communication skills with all members of the multidisciplinary team. 4. To acknowledge and respect language and cultural differences of patients/serviceusers, their support networks and colleagues. 5. Communicates to patients/service users and relatives compassionately, in aconfidential and sensitive professional manner. 6. To provide clear, concise, and accurate electronic and paper records of the care givenin accordance with agreed policy and standards. Leadership 1. To promote high quality care through the role modelling of professional behaviours. 2. To contribute to team success and excellent standards of care by challenging othersconstructively, upholding HSCs CARE Values. 3. To advocate on behalf of patients and service users 4. To take accountability for own decisions and actions utilising reflective practice. Planning and Organisation 1. To organise own workload and tasks, demonstrating the ability to recognise changingpriorities and adjusting plans accordingly. 2. To coordinate and delegate activities to junior colleagues within agreed systems ofworking. Policy and Service Development 1. To positively engage with service developments and contribute to changes to policiesand procedures. Partnership Working 1. To demonstrate an understanding of and apply the principles of the CARE Values. 2. To contribute to effective team working through communicating with others in aconstructive way offering support and assistance and making suggestions for theimprovement of care delivery. 3. To attend and participate in team meetings. 4. To actively work collaboratively as part of a multi-disciplinary team with a range ofprofessionals from other service areas who may be participating in the care of apatient/service user. To have an awareness of cost implications and assist in the management of resourceswithin the service area e.g equipment and supplies. 3. To ensure the safe and responsible use of equipment. 4. To take responsibility for maintaining own professional development, identifying owndevelopmental and revalidation requirements (if applicable), ensuring completion oftraining in accordance with the Statutory, Mandatory and Essential to Role TrainingPolicy. 2. To participate in supervisory one-to-one meetings and HSCs Values Based Appraisalprocess, developing plans to meet personal and organisational objectives. 3. To demonstrate the ability to develop new knowledge and skills, applying this topractice and sharing with colleagues as appropriate. 4. To participate in the induction, orientation and development of new staff, studentsand any voluntary workers. 5. To actively contribute to an effective learning environment and support others in theirlearning, participating in formal Supervisor roles as appropriate.
