Hospital at Homeis an exciting service that delivers acute-level care in the comfort of a patient’s own home. Working as part of a multi-disciplinary team, you will play a vital role in reducing unnecessary hospital admissions, supporting early discharges, and enhancing patient outcomes through personalised care.

Main Duties and Responsibilities
• Provide acute-level care in patients' homes, delivering safe, effective, and evidence-based clinical interventions in line with individual care plans.
• Assess, plan, implement, and evaluate carefor patients with a range of complex medical conditions who would otherwise require hospital admission.
• Undertake clinical assessments and observations, including NEWS2 scoring, Phlebotomy, Wound care, IV therapy.
• Use remote monitoring technologies to monitor patients’ observations and symptoms at home, enabling early identification of deterioration and timely clinical intervention.
• Support patients and carers in using monitoring equipment confidently, and integrate remote data into care planning through effective communication with the wider multi-disciplinary team
• Work autonomously and as part of a multi-disciplinary team, including GPs, nurses, AHPs, pharmacists, and social care professionals to deliver integrated, person-centred care.
• Support early discharge from hospitalby assessing suitability for Hospital at Home and ensuring smooth transitions of care.
• Educate patients and carerson managing health conditions, medication administration, and signs of deterioration, promoting independence where possible.
• Participate in regular clinical supervision, training, and team meetingsto maintain professional competencies and promote shared learning.

What is Staff Bank?

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

If you are joining our Trust in a fixed term role, please indicate on your New Starter Paperwork that you wish to join our staff bank.

· The post holder will be providing patient care in patients own homes, and able to travel independently across sites.

· Be computer literate and able to utilise technology to support the remote monitoring of patients, under the supervision of senior colleagues.

· Assist development of the service through reflective practice, clinical supervision & audit.

· The identification of suitable people referred to the service for planned early discharge from hospital or for the prevention of admission to the acute sector or long-term care.

· To be responsible, without supervision, for the holistic assessment of patient care needs.

· To be able to recognise deteriorating patients and use the appropriate patient pathways.

· To handover daily to the nurse in charge of each shift.To ensure timely and accurate handover of patient care, ensuring written records are consistent and completed punctually.

· Removal of drains, changing drainage chambers

· Palliative care support – syringe drivers, symptom management, family support, controlled drug administration

· Suture/Staple removal

· Wound care – Vac Therapy

· Management of constipation

· Pressure Ulcer Management – Pressure Ulcer Equipment

· 02 Therapy

· Zoladex Administration

· Referrals to other appropriate services e.g. Social Services, Specialist Nurses

· Management of Chemotherapy Patients i.e. discontinuation of ambulatory Chemo pumps

· Mentoring students/new staff

· Teaching students, patients and carers

· Developing IT skills/documentation

· Holistic Care or Diabetes

· Eye drops
