Description
We are seeking a dedicated and compassionate Community Support Worker (Scale 4) to join our team. In this role, you will play a crucial part in empowering individuals within our community to achieve their goals and enhance their quality of life. As a Community Support Worker, you will provide tailored support to individuals facing various challenges, including physical disabilities, mental health issues, and social isolation. Your responsibilities will consist of offering practical assistance, emotional support, and guidance, enabling clients to navigate their daily lives effectively. You will also actively promote well-being and independence through tailored care plans while fostering positive relationships with clients, their families, and other stakeholders. Responsibilities
Provide personalized support to individuals in their daily activities and routines.
Assist clients with personal care, including bathing, dressing, and grooming as needed.
Develop and implement individualized care plans in collaboration with clients and their families.
Facilitate community engagement and social activities to reduce isolation and enhance social interaction.
Encourage and motivate clients to develop independence and life skills.
Monitor and report on clients' progress, changes in needs, and any concerns to the relevant authorities.
Work collaboratively with healthcare providers, social workers, and other professionals to ensure comprehensive support.
Requirements
Proven experience working in a support role or similar capacity within the community.
Strong understanding of the challenges faced by individuals with disabilities or mental health issues.
Excellent communication and interpersonal skills with a compassionate approach.
Ability to work independently and as part of a team in a multidisciplinary environment.
Flexible and proactive attitude towards problem-solving and adapting to changing needs.
Relevant qualifications in health, social care, or a related field preferred.
Willingness to undergo background checks and adhere to safeguarding policies.

Pay Per Hour: £14.84
