We’re looking to recruit a Service Manager within the Early Intervention and Prevention Hub and

Hospital Social Work Team- someone who can bring energy, expertise and vision to our service.

Our “Strengths-Based Model of Practice” supports a multidisciplinary approach to enabling and supporting people who live with issues caused by disability, frailty, and ageing.

This role combines strategic oversight with partnership working with health colleagues in both acute and community health settings, and the voluntary sector ensuring the service is represented as working in an integrated, collaborative, cost efficient and responsive manner.

As Early Intervention and Prevention Hub and Hospital Social Work Service Manager, you will take a lead role in driving operational excellence, shaping service developments and ensuring high standards of professional practice across the service area. You will be responsible for supporting Team Managers managing complex risk, championing quality and ensuring our services remain responsive, effective and innovative in the face increasing demand.

The ability to greet customers through the medium of Welsh is a requirement for this post.

Protecting children, young people or adults at risk is a core responsibility of all council employees.

An Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.

The council’s Hybrid Working Policy applies to this post. This provides a framework for establishing how you will undertake working hours between your home and the office.
