Summary
The States of Guernsey are looking for a Reablement Support Worker (Band 3) to join the team at Adult Community Services.
The aim of the Reablement service is to assist individuals who are returning home from hospital or to work with them to prevent admission to hospital. The team assist by helping people to regain their independence through Reablement support. Reablement is short term support which gives the opportunity, motivation, and confidence to relearn or regain some of the skills required for daily living, which may have been lost because of illness, accident or disability.
