We are looking an experienced Band 3 Healthcare Assistant who is organised, enthusiastic and self-motivated to deliver excellent care. You will be joining our existing and well established Heart Failure Team within a community setting and with close links with our acute partner, Cardiac Rehab and Palliative Care Team. The team is also well supported by a consultant cardiologist and administrators

Experience of working in a health care setting and phlebotomy are essential for this post. You will need to have excellent interpersonal and communication skills with an ability to use your own initiative as well as work effectively as part of a team. You must be flexible and have a positive approach to your work. The post holder will be required to communicate effectively with patients, family, carers and members of the public and staff.

For the right applicant there is the opportunity to develop as a registered nursing associate.

The role will include, but is not limited to:

Assisting the Community Heart Failure Specialist Nurses in the care planning, assessment, monitoring and review process of patients referred to the Community Heart Failure Service.

Independently visiting patients in their own home to carry out a basic heart failure assessment and undertake simple clinical tasks such as weight, blood pressure, pulse and oxygen saturation monitoring, venepuncture sample collection, skin care inspection and prevention of pressure sores as delegated under the guidance of the Community Heart Failure Nurse.

The role involves working closely with GP’s, other members of the integrated community teams, and with social care partners. As such you will be required to have excellent communication skills and be confident to work using your own initiative as well as being an excellent team player.

A car driver and owner are essential to this role as you will cover a wide geographical area visiting people across West Hertfordshire.
