Summary

You will be committed to improving lives for our service users and have an excellent interpersonal and communication skills. You will be pivotal in supporting and empowering the health promotion and health needs of our service user.
Our service users all live in their own homes and have Learning Disabilities, some also have health health diagnosis in supporting the health needs and health promotion to support and empower people with a Learning disability.
Each service user is staffed by a dedicated team. This is an excellent opportunity to practice within the challenging yet rewarding field of community based learning disability services whilst working alongside a multi-disciplinary team.
This Team leader post is a full-time Monday to Friday role. We have an on call system in place to support the health care assistants in their roles. The on Call system is 24 hours per day 365 days per year.
This post will require you to become part of this on call rota. This will have a up lift of salary, depending on how many you do and if called you will receive overtime payment for out of hours. As this is a community post being a driver is essential for this role and have 2 years experience as a Band 5 Nurse
This role requires you to demonstrate your commitment to working proactively and in partnership with people with learning disabilities. You will use your professional knowledge and experience to support individuals within the community.
You will provide support and sometimes work directly with people with learning disabilities in their homes. This may involve working with their families, healthcare assistants, other professionals. You will be a dedicated Team Leader for at least two services.
All practice should be based on the principles of Valuing People—rights, independence, choice, and inclusion. You will work with individuals and their carers to help develop, implement, and review person-centred care plans and risk assessments. These will outline the support each individual needs to live successfully in the community.
As a Team Leader, you will also support healthcare assistants who work daily with our service users. You’ll provide clinical expertise, and guidance to ensure that the specific needs of each service user are met. You will carry out training, supervision, sickness management. You will also assist with shift cover using e-roster, and support staff with a range of issues as they arise.
We provide a 24/7 on-call service, 365 days of the year. This service is shared equally between team leaders and management. The on-call rota is planned one month in advance. Each on-call shift runs for 24 hours, from 8:00am to 8:00am the following day.'
