Location: SweetTree Home Care Services - Coleridge House, 1 Coleridge Gardens, London NW6 3QH
Schedule: Monday - Friday, 9am - 5pm. 37.5 hours weekly.
Salary: £27,000 - £30,000 per annum, pro rata basis

Role Overview: As the Assistant Care Manager, you will support the Dementia Service Live-In Manager in delivering high-quality care and support to our clients. You will assist in managing the Support Worker team, ensuring compliance with SweetTree’s Quality Assurance markers, and contributing to the achievement of our company KPIs.
Key Responsibilities:
Ensure a high-quality, client-centred approach, focusing on goal attainment and enhancing quality of life.
Audit the delivery and accuracy of support as outlined in Support Plans, including outcome monitoring and medication management.
Review Daily Support Records and escalate any concerns to the Support Manager.
Prepare and deliver client care reports to third parties (e.g. Solicitors, Case Managers, Local Authorities) as needed.
Assist the manager in implementing ST Connect within the live-in service.
Conduct spot checks and client reviews alongside the Live-In Manager.
Participate in needs assessments and prepare care and support plans, ensuring they are regularly updated.
Support the Live-In Manager with administrative tasks related to client and support worker documentation.
Maintain and update the care certificate log for new support workers during their probation period.
Conduct competency spot checks and probation reviews for support workers.
Proactively develop ongoing Learning & Development for Support Workers and assist in performance management.
Assist in interviewing candidates and lead cross-service interviews as required.
Facilitate training sessions for Support Workers as needed.
Coordinate package deliveries and shift schedules effectively.
Answer the New Enquiry Line as required.
Act as a backup for emergency client support during uncovered daytime shifts.
Participate in the on-call out-of-hours service rota as needed.
Answer telephone calls in the SweetTree office actively.

Essential Experience, Skills, and Attitudes:
Experience working with individuals requiring Live-In care.
Experience supporting individuals with dementia.
Ability to work within a team in a fast-paced, pressured environment.
Desirable experience as a support worker within the community.
Desirable administrative or IT qualifications.
A proactive attitude that embraces challenges.
Strong interpersonal skills with the ability to motivate a remote workforce.
Excellent dispute resolution skills and composure under pressure.
Experience working positively in a diverse environment.
Effective verbal and written communication skills at all levels.
