Summary
We have a 24 hour Part-time, Permanent opportunity for a passionate and dedicated individual to join our new service in Ceredigion as a Floating Support Worker, where you will provide community support to individuals who have mild learning disabilities.
It is essential that the successful candidate holds a driver's license and has access to a car.

Your day will consist of:
Conducting needs assessments and person centred support plans
Provide practical and emotional support to the individuals you support
Provide support around housing challenges, mental health issues, substance misuse issues and other complex needs.
Assisting with sustaining and securing suitable accommodation
Maintaining accurate and up-to-date records
Developing your networking skills by working in partnership with external agencies
Delivering workshops to the individuals we support.
Who we are looking for:
Previous experience in a similar role
A passion and commitment to supporting vulnerable individuals and promoting independence and wellbeing
Organisation and time-management skills
Ability to work independently as well as part of a team
A non-judgmental and empathetic approach
What are the pay and benefits?
Part-time - 24 hours, permanent position
Competitive salary of £24,531.00 FTE (£12.75 per hour)
Work pattern: predominantly Monday – Friday office hours although flexibility is required to meet the needs of our customers.
An enhanced DBS is required for this role which we will pay for
26 days (pro rata) holiday entitlement plus Bank holidays
