Summary
Accept and make referrals across a wide range of agencies while building proactive team links within the wider PCN team. Meet individuals one-to-one in appropriate settings, offering time and space to focus on what matters to me. Build trusting relationships through a holistic approach, addressing wider determinants of health. Use motivational interviewing to support personalised action plans and SMART goals that empower individuals to improve their wellbeing. Recognise when it is appropriate or necessary to refer clients to other professionals or services beyond the scope of the role. Link individuals to local community groups and services, encouraging meaningful participation. Proactively develop partnerships with local agencies to raise awareness of social prescribing and reduce pressure on statutory services. Record data using recognised outcome measures - e.g., Wellbeing Star, SWEMWBS - and submit monthly reports. Provide one anonymised case study per month demonstrating referral concerns, goal setting and outcome actions to support service evaluation. Maintain accurate, timely records of client interactions, outcomes, and referrals within the expected timeframe to support effective service delivery.
