Summary
The post holder will work as an autonomous practitioner within an interdisciplinary Neighbourhood team providing rapid and expert professional assessment and treatment interventions to patients either in their own homes or other community settings to avoid inappropriate hospital admission or to facilitate early discharge. This will be achieved through a combination of hands on expert clinical skills as an Occupational Therapist with knowledge to treat, educate and support patients; and by working with patients and their families to assess, plan and implement appropriate packages of care and treatment Provide case management for a designated caseload of patients who have immediate and / or complex care needs. Triage patients to ensure prioritization of clinical need. Work holistically with all team members, carrying out joint visits and actively participating in the integration of services increasing the knowledge of all team members through case discussion and joint working For full details of the duties and criteria for the role please refer to the job description and person specification attached.
