Summary
We are working closely alongside a Local Authority in Lincolnshire to assist with the appointment of a Leaving Care Worker to work in a residential care setting.
We have both regular shifts (37 hours per week) and bank shifts available. Please note that we don't offer sponsorship, and the worker should be able to work full time without restrictions. We would need 5 years work references checked out before you start the shifts.
Rate of Pay: £17.30 - £22.48 per hour
Summary:
The Leaving Care Worker will serve as a vital point of contact and support for young people transitioning from care into adulthood. The role involves fostering relationships, providing practical support, and contributing to strategic planning aligned with the Children in Care and Care Leavers Strategy.
Responsibilities:
Manage a caseload of young people, providing tailored support and guidance.
Work with young people to build life skills including budgeting, self-care, and home management.
Act as a Lead Professional when appropriate and coordinate inter-agency plans and interventions
Ensure compliance with safeguarding policies and legal requirements.
Attend relevant meetings such as Case Conferences and contribute to outcome evaluations.
Qualifications:
NVQ Level 3 in a relevant discipline or equivalent
Essentials:
Valid driving licence and access to a vehicle
Familiarity with legislation related to children in care and care leavers.
Ability to assess needs, prepare reports, and implement outcome-based plans.

You should be available to work immediately or at a short notice.
