Summary
To provide day-to-day clinical leadership for the multi-disciplinary and multi-agency service to deliver effective, efficient, and high-quality Primary Care services where those Primary Care services need to be provided in people's homes, care homes and those identified with unmet needs and/or complex ambulatory patients.
