Summary
You will contribute to a range of medicines optimisation and patient safety initiatives. Your day-to-day work will include: Supporting medication reviews and medicines reconciliation Helping patients understand and use their medicines correctly Promoting safe and effective repeat prescribing processes Assisting in audits, service improvement, and prescribing projects Liaising with local pharmacies and secondary care to ensure continuity of care Playing an active role in reducing medicines waste and improving outcomes Youll be a key part of our multidisciplinary team and will benefit from the opportunity to grow your skills and knowledge in a collaborative, supportive environment.
