Summary
Salary: £185.40-£216.30 per session depending on experience Tenure:24 Months Hours:4 sessions per week-2 on a Wednesday and 2 on a Friday- this can be worked by one person or can be split between two people Location: Scarborough, Whitby & Ryedale area. Duties and Responsibilities: The duties and responsibilities to be undertaken may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Medical Director or Board of Directors, dependent on current and evolving workload and staffing levels. Clinical Responsibilities: Assess the healthcare needs of complex patients with both defined and undifferentiated or undiagnosed problems, carrying out holistic face-to-face patient assessments and developing comprehensive management plans based on Comprehensive Geriatric Assessments (CGAs) Make professional, autonomous decisions, with confidence to escalate when required Lead the clinical management of patients as appropriate, including initiating investigations, reviewing results, liaising with secondary care, and making referrals to other providers and services as appropriate Contribute to Advance Care Planning and Treatment Escalation Plans through completion of CGAs & ReSPECT forms, including best interest discussions and decisions where required Contribute to the triage of referrals to the Virtual Frailty Ward Record clear and contemporaneous computerised consultation notes to agreed standards Compile and issue computer-generated acute and repeat prescriptions Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance) Work with and support other members of the MDT, including but not limited to Advanced Complex Care Managers, Nursing Associates, Specialist Pharmacists, Physiotherapists, Urgent Care Response Paramedics and other Frailty GPs. Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health, British Geriatrics Society and local NHS trust Participate in national frailty training and maintain a high volume of CPD in Frailty-related conditions, with the aim of obtaining the Diploma in Geriatric Medicine Please see attached Job description for more information.
