Job Purpose - Monday- Friday 9-5
· Children’s Continuing Care assessments and reviews for the eligibility of continuing care packages. These children may have life-limiting / threatening conditions and/or may be technology dependent.
· The post holder will be a designated resource to children and their families within the case load, therefore requiring liaison with multidisciplinary teams, other community services, and working across organisational boundaries within the London Borough of Barnet.
· The post holder will be responsible for ongoing case management of care packages for the child and families until they transition to adult services, or they are no longer eligible for Children’s Continuing Care.
To work within the remit of the National Framework for Children and Young People’s Continuing Care and participate in peer review of assessments and reviews with colleagues across NCL.
To keep accurate records of all clinical activity using the agreed system.
Liaise effectively with specialist staff at tertiary centres when the care of a child is shared or complex and inform any external stakeholders of a change in a child’s needs
To attend multi-professional meetings relating to the children on your caseload.
To receive and process referrals and participate in shared decision making on criteria eligibility, referring on to appropriate services if criteria not met
To carry out risk assessments of community environments prior to the delivery of care and respond to outcome of assessment.
To order essential equipment required in the home where appropriate, organise safety checks and ensure equipment is maintained in keeping with guidelines, throughout its use in the home
