We are looking for a dedicated Care Coordinator to join our Doncaster team and help deliver excellence in care coordination and staff support.
Role Summary:
As a Care Coordinator, you will be the key link between our clients, care staff, and management. You will oversee care plans, schedule visits, supervise care workers, and ensure we meet both client needs and compliance standards. This role demands excellent organisation, people skills, and a flexible, proactive attitude.
Key Responsibilities:
* Plan and coordinate care worker rotas, ensuring timely and efficient service
* Conduct initial care needs assessments and write tailored care plans
* Monitor care delivery, adjusting plans as client needs change
* Ensure compliance with CQC regulations, safeguarding, and GDPR
* Provide support and supervision to care workers, including spot checks and training
* Handle client and staff queries, complaints, and changes to schedules
* Participate in the on-call rota and respond to out-of-hours issues as required
* Support recruitment, induction, and ongoing development of care staff
* Cover care visits when required due to staff absence or emergencies
Essential Requirements:
* Proven experience in a care coordination or supervisory role within domiciliary care
* NVQ Level 3 in Health & Social Care (or equivalent or working towards)
* **Must be a driver with access to a reliable vehicle**
* **Willingness to work flexible hours**, including early mornings, evenings, and weekends as needed
* Strong understanding of care sector regulations and CQC standards
* Excellent communication, leadership, and problem-solving skills
* IT literate – confident using scheduling systems and Microsoft Office
* Enhanced DBS check (or willingness to undertake one)
* Right to work in the UK
Desirable:
* Knowledge of Doncaster and surrounding areas
* Experience using care management software
* Ability to mentor and support junior team members
