Summary
You will work as part of a multi-disciplinary team to support the delivery of the Early Intervention and Prevention offer for young people and families, adopting a whole family strengths-based approach to reduce risk and increase resilience and improve outcomes for children and young people.
You will deliver and lead core functions of the Wakefield Families Together Youth and Family Hub delivery model, working with partner agencies to improve outcomes for children, young people, and families.
You will be supporting the planning, organisation and delivery of high quality, evidence-based early intervention, and prevention services, in partnership with colleagues from voluntary and statutory services, to improve outcomes for children and families and reduce the need for more targeted or specialist interventions.
You will be coordinating the preparation and response to Early Help referrals received via the Team Around the School or Team around the Early Years process to support the needs of a child, young person, and family and to gather relevant information to enable appropriate allocation.
You will facilitate the development and implementation of an agreed plan of support for children and you will be contributing to the delivery of effective interventions for change using evidence-based programmes and multi-agency approaches.
You will promote and support participation of young people in all aspects of service design, delivery, and review.
You will support the council in meeting its statutory duties under the Children Act 2006 with regards the provision of early childhood support.
You will report on outcomes as part of the Children and Young People’s Performance Framework.
You will be ensuring that services are delivered in compliance with relevant legislation, local and national policy, and guidance.
