Summary
The post holder will be responsible for providing administrative and practical oversight of all referrals in and out of CAMHS Acute (inpatient and crisis/intensive home treatment) ensuring a professional, timely and robust response.Oversee the access and discharge points to the service - managing data, documentation and information sharing.Create a better working relationship with external services (e.g, social care, Provider Collaborative, community CAMHS teams, LD/A services and education); by actively promoting integrated health/care professional working internally and externally keeping the young person central to their actions to ensure smooth transition and communication between services.Ensure a smooth patient flow throughout the whole pathway, ensuring patients lengths of stay are no longer than clinically necessary, whilst ensuring quality of care and patient experience is upheldLiaise with external agencies to ensure that young people's needs are met in the community to prevent unnecessary hospital admissions as well as re admissions to inpatient care.
