Summary

Job Purpose To work alongside Nurses, Physiotherapists, community support workers and administrators to provide assessment and treatment intervention to patients with a confirmed respiratory diagnosis or requiring home oxygen. To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values. To maintain records as an autonomous practitioner working with supervision Dimensions You will be working in a variety of community settings including health centres, community centres, a patients own home or residential home following treatment plans and monitoring the patients condition/progress Undertake clinical observations such as BP, temp, oxygen saturations capillary blood gases etc Delivering Pulmonary Rehabilitation sessions Managing a caseload of patients with a confirmed respiratory diagnosis. You will participate actively in the services training strategy, including in-service training and multi-agency training. Working in conjunction with and under supervision of senior clinical professionals. Able to fulfil physical demands of role, including manual handling applications. Key Responsibilities Clinical 1. To undertake individual treatment and interventions with clients under the supervision of qualified staff. 2. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role. 3. To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user. 4. To promote wellbeing, for service users through Pulmonary Rehabilitation. 5. To attend and contribute to team meetings and case discussions using effective communication. 6. To maintain comprehensive and accurate records in line with employers polices. 7. To promote health and wellbeing with all citizens. 8. To undertake a range of MDT competencies, as required by the service. 9. To operate under the guidance of clinicians and only within the scope of AP practice. Managerial and Leadership. 10. To provide support to other Community Support workers as appropriate. Education and Development 11. To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines). 12. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also to keep an active up to date CPD portfolio. Clinical Governance. 13. To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations. 14. To have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties. 15. To actively promote equality and diversity within service delivery and peoples individual rights. To participate in any other agreed duties that may be reasonably required in addition to those above.
