Summary
The Team Leader role combines support work with managing a team as well as carrying out the same tasks as a Support worker. You will have responsibility for supervising staff and volunteers across a cluster of services, encouraging and inspiring them to offer the highest standards of support.
To ensure all aspects of care and support to the service users is maintained at a consistently high level. Providing emotional and practical support to them on an individual basis, to enable active participation in the community. To ensure that individual programmes of care are planned, executed and monitored, so that a high quality of service is provided. To work as a member of the staff team and assist in the supervision and professional development of members of the team and to carryout duties as delegated by the manager.
Principal Duties & Responsibilities
1. Ensure individual assessments, plans and guidelines are monitored and reviewed, by giving sufficient knowledge, capacity and negotiating skills to assess level of need with the service users and compile a suitable stimulating plan wherever possible, to ensure young people get the best from their stay.
2. To oversee and participate in the development of Assessment of Needs, Support Planning, Risk Assessment etc. and continually offering a person centred approach. To oversee all service user’s files/documentation to ensure key-workers are keeping it up to date.
3. Assist in ensuring all service users or their relative/advocate are aware of, and understand the Complaints Procedure and that it is rigorously followed.
4. Ensure the maintenance of good relationships with individuals, team members, relatives, stakeholders and the wider community.
5. To be able to work on own initiative and take responsibility during shifts within the project.
6. Ensuring the Operational Policy is adhered to in all respects.
7. Ensure all proper records are maintained in accordance with the appropriate Registered Acts & Regulations.
8. To undertake work as reasonably requested by the Project Manager.
9. Present issues verbally or in report form in a variety of settings in a professional manner and confidentially.
10. To maintain professional relationships and boundaries with health professionals and CQC registration and compliance Team, and other external providers.
11. You’ll also need to ensure that all documentation completed by your team is comprehensive and up-to-date, whether that’s risk assessments, care plans or financial records. This will require you to be confident in using and training others in IT systems and databases.
12. Ensuring the home environment remain clean and all staff are adhering to Health and safety procedures.
13. To be a role model to the team and demonstrate excellent practices.
14. To undertake any other duties as requested by the Manager.
Performance
1. Be required to sleep-in as and when required at the service.
2. Be aware of criteria and referral routes from the local authority/social care team.
3. To participate in activities within and outside the home, including cooking, shopping, cleaning, where this is appropriate to an individual’s plan of care, to undertake personal care.
4. To be aware of the steps to be undertaken when medical emergencies arise.
5. To be aware of personal responsibilities regarding health and safety at work and fire procedure.
6. To undertake staff supervision, both formally and on a day to day basis.
7. To be responsible for administering medication ordering storing and replenishing. Be accountable for your actions.
8. To assist manager in managing attendance and managing performance of staff team.
9. To participate in internal and external audits and monitoring. To be involved in carers/multi-disciplinary meetings and reviews, as directed by the manager.
10. To manage finances in the house and service user’s monies where necessary. To adhere to budgetary requirements.
11. To work with the manager to identify areas that need change/improvement in current practice and support change when areas of concern are highlighted.
12. Provide written reports as and when required. To be able to use computer, access email, write reports, documents events as they occur this could be in written format or computerised e.g. accidents/incidents.
13. Develop a wide network of professional and local people to share ideas and opportunities.
14. To support service users to develop and achieve their ambitions.
