Summary

Job Purpose To work as an autonomous practitioner, you will undertake comprehensive assessments to identify the appropriate service for the clients referred. To carry out profession specific assessment and evaluation as required. To work within the MDT to plan and develop discharge plans for adults with a wide variety of complex physical and mental health needs To be responsible for continuing the on-going development of high standards of clinical practice with evidence-based focus within the service. To work, as required within the integrated care and Front Door Integrated Discharge services, reacting to emergency health and social care referrals with the aim of preventing admission to hospital. Dimensions This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness. You will be required to work primarily with the Queens Medical Centre but with potential to work across different bases and community settings which may include clients own home, residential, nursing homes or hospital. You may be responsible for a designated caseload of clients You will be required to participate actively in City Cares training requirements. You will supervise, educate, and assess the competency of junior staff, support staff and students as required. Key Responsibilities Clinical 1. To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. 2. To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment. 3. To undertake the comprehensive assessment and evaluation of the patients Physical and Mental health needs to develop individualised discharge plans/rehabilitation/reablement goals. 4. To undertake specific professional assessments to ensure the best outcomes for clients 5. To use clinical reasoning and a wide range of skills and knowledge to devise specialised programmes of care. 6. To be responsible for coordinating the safe and timely discharge process from a variety of settings, including hospital and community beds. 7. To undertake comprehensive evidence-based risk assessments developing risk management plans. 8. To provide specialist advice, education, and mentorship to other members of the team and colleagues regarding the clinical management of clients. 9. To actively engage with families and carers in order to ensure the best outcome for the service user. 10. To demonstrate flexibility, prioritise effectively and time manage own clinical workload. Be able to delegate appropriately. 11. To lead in regular team meetings and review meetings regarding client caseload. 12. To keep accurate and timely service user records to ensure continuity of care. 13. To participate in the supervision framework as a supervisee and a supervisor, including the appraisal process. Professional 1.To maintain a high standard of care to clients and carers, demonstrating a commitment to evidence based practice. 2. To be responsible for own Continuing Professional Development maintaining an up-to-date portfolio. 3. To work to relevant Code of Professional Conduct. 4. To contribute to Clinical Governance agenda and apply Risk Management strategies. 5. To contribute to implementation of service developments utilising enhanced knowledge and skills. Organisational 1. To support the Team Manager in implementing and evaluating targets and objectives to assist in the ongoing development of the service. 2. To advise the Team Manager in relation to clinical needs of the client group. 3. To take delegated responsibility for management duties as required. 4. To always ensure the health & safety of self and others in accordance with Health & Safety legislation and City Care policy and procedures. 5. To use information technology daily and participate in identified data collection and statistics and maintain clear, concise, and timely records in line with the service policy. 6. To participate in the Lone Worker Tracking procedure as applies to the team. 7. To act as duty clinician for the process of referrals. 8. To undertake any other duties which may be reasonably required. Education and Leadership 1. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio. 2. To participate in the education of colleagues, junior staff, students, and multi-professional teams using excellent mentoring and presentation skills. 3. To assist in the training and development of others (new members of staff, students, and visitors to the service). 4. To keep up to date with IT skills in line with the modernisation of data collection records. 5. To participate in appropriate in-service training and individual learning with specific and measurable outcomes.
