Summary
There will be management guidance and training of the post holder. This will be managed on an individual basis and according to knowledge and training needs in accordance with local trust and national guidelines. There will be set competencies to achieve. To practice at all times within the nursing and midwifery council (NMC) code of conduct. To ensure practice is maintained according to National, local and trust policies and guidelines on coronary heart disease, the management of heart failure and surgical intervention. To give expert advice on management of cardiac patients including medication and further treatment options. To manage a caseload of patients. To take responsibility for own self development in liaison with line manager. To act as a clinical advisor on cardiac rehabilitation to all members of the multidisciplinary team. Participate in updating CHD guidelines, protocols in line with clinical governance guidelines. Promote rehabilitation patient autonomy as a focus of care. Make contact with patients during acute episode and perform risk factor assessments. Review patient notes to ensure all MI/Heart Surgery/ACS patients are seen for advice/support, and relevant investigations are requested in line with trust guidelines and cardiology teams. To participate in cardiac rehab Phases I III as per BACPR guidelines. To act as the patients advocate by discussing treatment approaches, writing letters to consultants/GPs to maintain a good quality of life for patients while reducing risk factors. To see inpatients/outpatients for pre surgery/investigations advice support To provide a support line for rehabilitation/RACPC/Angiography patients and relatives to reduce anxiety for all clients involved/beneficiary of cardiac rehab service. To work closely with the Cardiac Rehabilitation fitness instructor maintaining a team approach to high standards of care. Maintain a safe and healthy environment for patients/staff/relatives within the exercise programme. Have and maintain life support skills Advance clinical practice in line with service needs.
