Young person requires support with enjoying their hobbies
Preferred days/times:
8.30-3.30 Mondays
Tasks/duties:
To take the client out for the day, for example to the farm.
7 hours per week
Client main diagnosis:
Smith-Lemi-Opriz Syndrome and associated Learning Disabilities.
Focal epilepsy – Takes medication – under control no seizure for several years
Requires disabled toilet but doesn’t like the door locked – PA to stand outside
Needs to rest legs while walking so will take periodic breaks
Communication:
The client uses vocal communication and Makaton – if they go out they will need help as they cannot read and write
Can become frustrated when cannot be understood however does not exhibit challenging behaviours
Client enjoys:
going to the farm as they love animals, they also like cars, the cinema, discos, music, cooking and swimming - they get in and out of the pool with assistance
PA to have their own vehicle and full, clean, driving licence. Mileage payable of £9 per week.
No pets
