Summary

To assess, plan, implement and evaluate the nursing care and support requirements for children with medical, health needs.2. Provide skilled paediatric nursing care to children and young people within their own home, or other applicable venues such as outpatient clinics, to ensure families are supported in the care of their children at home.3. Ensure continuity of care is established and appropriate support available for children, young people, and families, with the aim of preventing unnecessary admission or attendance at hospital and facilitating earlier discharge where appropriate from both local and tertiary hospitals.4. Maintain excellent communication with the child and family on the caseload and as their advocate ensure that they have ease of contact with the team appropriate information and support. Records should reflect the childs care, present condition and progress and be available to colleagues5. Maintain the safety of children and young people by ensuring robust care plan are in place with the supervision of the senior team.6. To liaise with children and their parents/carers along with other agencies who are involved in their care to promote a person-centred outcomes approach to supporting their needs.7. Promote the health, comfort, dignity, privacy and quality of life for children andyoung people.8. To support the provision of acute care to children and young people following agreement of care from the wider teams.9. Ensure prescribed nursing care is carried out in line with local and national policies including administration of SC/IV/CVAD IV treatments, taking of blood samples or specimens as required.10. Identify those children and families with continuing care / complex needs and plan programmes of care in accordance with either the local agencies or continuing care framework.11. To provide theoretical training and competency-based assessments in relation to nursing interventions for staff working in partner agencies including schools (mainstream and special needs), colleges and early years settings.12. To support the Leadership Team in developing service improvements to ensure the health needs of the children and young people who use the service are met.
