The primary role of the community Nursery Nurse role involves visiting infants in their own homes or at alternative venues such as clinical settings or community spaces across Hampshire as delegated intervention by the infant mental health team Health Visitors and Early Years Practitioners.The role includes working in partnership with internal and external colleagues and partners showing knowledge and understanding of the parent-infant relationship and contemporary issues which impact on child development, infant mental health, and wellbeing. The role holder will work in partnership with infant mental health team members to offer evidence-based/informed approaches that promote infant-led play, enhance language development, improve infant-parent interactions, and foster the infant's developing attachment and the bonding process with parents. The role The role holder will promote and support health and wellbeing of infants and their families through individualised health care plans with the oversight of a team health visitor. The role holder will have a comprehensive understanding of infant-childhood development with an ability to apply this to practice and service delivery. The role holder will ensure the quality-of-service delivery to infants and their families is aligned with the commissioned requirements of the Healthy Child Programme 0-19yrs and the Trust vision and values. The role holder may complete delegated and contribute to components of the healthy child programme, monitoring quality and compliance and reporting risks by exception to the specialist infant mental health visitor lead. The role holder will provide a continuum of support according to need, improving outcomes for children by reducing health inequalities. To support the Early Help agenda to ensure early intervention. Work within Hampshire Safeguarding Children's Board and the Trusts policy and procedures. The role holder will be responsible for the quality and management of their RiO electronic patient records, RiO mobile and electronic diary. To maintain confidentiality and adhere to local polices at all times in line with Trust Policy. To ensure accurate and concise record keeping relating to both infants and parents as well as documenting interactions with external agencies where appropriate To recognise and promote all aspects of culture diversity, devising infant mental health care plans as guided by infant mental health visitor, that respond positively to ethnic awareness with sensitivity, where necessary Support parent with the infants physical and emotional needs. Working alongside current guidelines regarding sleep, settling, managing crying, keeping in mind, infants brain development and emotional needs. Role modelling- safe handling of infant. Awareness of safeguarding protocol and escalating and recording as needed. To work within Hampshire Safeguarding Children's Board and the Trust Safeguarding Policy and Procedures. To contribute to interventions identified through the child protection plan as delegated by the Health Visitor, and support children and families where there are identified vulnerabilities. To deliver a safe and effective service using service user feedback to improve patient experience and outcomes. To support Health Visitors with integrated working; developing and maintaining relationships with statutory and non-statutory agencies. To liaise regularly with other professionals involved in the infant's care To always treat team colleagues with respect, creating a positive role model and team player Establishing communication networks with other professionals and agencies to ensure a comprehensive and consistent approach to client care is delivered. Health promotion in line with current HCP guidance i.e., ICON, Safer sleeping, and introduction to solids. Working with infants and toddlers up to two years. The role of the postholder will be to support the inter-disciplinary team in meeting the needs of infants and their families by undertaking delegated duties that support and enable family centred care. The role includes ensuring that there is appropriate communication with other inter-disciplinary professionals involved in the care of the infant and family. An important facet of this role is to work within the ethos of 'think family'. A family approach can be effective in helping families and secures better outcomes for children (including unborn infants), adults with care and support needs, children, and their families; therefore, consultation and partnership working with all professionals (internal and external) involved with the infant and family is a key part of this role. The role has a degree of autonomy. The postholder will be required to work as an integral part of the inter-professional team. The post holder should be able to prioritise their own work and manage their own electronic diary. Care plans are developed in partnership with service users, the postholder will have the support and the review of a team health visitor to oversee parent and childcare planning. To provide parenting advice including play, development, sleep and behaviour management Maintain accurate and contemporaneous records in accordance with Trust policy. To identify opportunities for innovation and support Health Visitors in multi-agency working to enable delivery of the Healthy Child Programme. To obtain service user feedback, participate in service user forums and support the Health Visitor to improve patient experience with engagement and service improvement. To ensure that services provided are to the required Trust quality and standard and in accordance with the Healthy Child Programme raising any risks to the Clinical Team Leader To ensure electronic patient records and data-collection systems are used appropriately, accurately and effectively to record and manage information which will be used to support service delivery and to report on Key Performance Indicators, supporting business need. To contribute to programmes of learning for new staff and students. To utilise training, development and the appraisal process to ensure access to appropriate learning opportunities and undertake statutory and mandatory training as required by the organisation. To adhere to the Clinical Supervision and Safeguarding Supervision policies. To ensure that Management Supervision is accessed every 3 months as a minimum or as required.
