Summary
RESPONSIBLE TO: GP Partners (Clinically),Practice Manager (Administratively) We strive to promote the best evidence based care for our client population in the management of long term conditions and the proactive provision of preventable health care. Key clinical responsibilities: To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the practice and primary care team. Assess, plan, develop, implement and evaluate treatment plans that promote health and wellbeing, and for patients with long term conditions. Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long term condition. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols. Work with patients in order to support adherence to prescribing treatments. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote patients to live healthy, and apply principles of self-care. Support and manage health needs of women presenting for family planning and cervical cytology consultations. Recognise, assess and refer patients presenting with mental health needs in accordance with the NSF for Mental Health. To be aware of local mental health care pathways, and referral/support systems. Implement and participate in vaccination and immunisation programmes for both adults and children. Advice, support and administer vaccinations where appropriate for patients travelling abroad. Meet the needs of patients presenting with opportunistic wound care. Where the post holder has completed an approved prescribing qualification for the role and the practice determines that this is a requirement: to prescribe safe, effective and appropriate medication as defined by current legislative framework and within personal competence and agreed formulary boundaries.
