Summary

The Coach will work closely with the other members of the Primary Care Team in the management decision-making of care and service provision for individual patients of low to medium complexity, who will usually have one or more long term conditions. This will include: Building relationships with the PCNs multi-disciplinary team and GP Practice staff, promoting health coaching and its role in supported self-management of personalised care, actively participating in meetings as well as identifying and addressing health inequalities in the PCNs population. Accepting referrals from the GP Practices. Managing and prioritising a caseload in accordance with the health and wellbeing needs of their population, through a non-judgemental approach that is based on strong communication and negotiation skills. Where required, and as appropriate, the Health and Wellbeing Coach will refer people back to other health professionals within the PCN. Working alongside the multi-disciplinary General Practices teams within the PCN to effectively plan and implement a high-quality Health and Wellbeing Coaching service, as part of a broader primary care team. Providing personalised coaching and support to individuals to allow them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently through: o engaging and motivating patients through multiple sessions to identify their own needs and set goals, supporting them to identify and implement their own personalised health and wellbeing objectives whilst focussing on what matters to them. o coaching and motivating patients through multiple sessions to identify their needs, setting goals, and supporting patients to achieve their personalised health and care plan objectives o providing interventions such as self-management education and coping skills. o supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses o working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing, and o increasing patient motivation to self-manage and adopt healthy behaviours Supporting one-to-one sessions on areas such as healthy eating and physical activity along with general health and wellbeing to support personalised care plans. Working within a multi-disciplinary team as well as our local VCSE (Voluntary, Community and Social Enterprise Sector) organisations. Working autonomously to support patients to manage their physical and mental health and wellbeing, build community resilience and make informed decisions and choices when their health changes. Delivering a universal and proactive offer of support to people with long-term physical and mental health conditions, building knowledge, skills and confidence to live well with their health condition. Playing an active role in Multi-disciplinary Team meetings if required (regular Practice and/or PCN meetings to discuss high risk and/or complex patients) by gathering information and being prepared to update the team on patient progress towards goals etc. (as per their care plan). Building and maintaining strong links with the voluntary sector, supporting the voluntary and statutory sector to network and improve partnership working Supporting the delivery of systematic self-care support plans for those with COPD, diabetes, asthma and multiple long-term conditions. Understanding the barriers for individuals in accessing support in the community and using this insight in developing community-based support, working as part of the wider social prescribing model. Contributing to and working with others to organise awareness-raising events for services that help support people to improve their health and wellbeing. The coach will have a key role in helping to raise the local populations awareness of the support, groups and opportunities available to assist them in achieving their health and wellbeing goals.
