Overview of post

This post will provide sub-specialty experience in Lung Cancer, advanced diagnostic & therapeutic bronchoscopy (including EBUS, navigational bronchoscopy, cryotherapy/APC), Pulmonary nodules and Pleural Disease, and would be suited to a Respiratory trainee as out-of-programme experience towards the end of specialist training or as post-CCT experience. The post is based in the Oxford Centre for Respiratory Medicine, at the Oxford University Hospitals NHS Foundation Trust. The post holder will be one of two Clinical Fellow posts that provide support for the Multidisciplinary Team (MDT) in the care and diagnosis of patients with Lung Cancer and management of pulmonary nodules. The post holder will also support the administrative responsibilities associated with regional and mandatory national cancer databases and delivery of the cancer waiting time targets. In addition to this clinical experience, the postholder will have the opportunity to participate in departmental education meetings and teaching within the Oxford Centre of Respiratory Medicine and to be involved in research projects relevant to lung cancer or pulmonary nodules.

At OUH we have recently established a Respiratory Early Diagnostic Service (REDS) comprising of 2 nurse specialists & an administrative pathway navigator alongside the lung cancer consultants and fellows. This exciting initiative provides us the potential to exceed the National Optimal Lung Cancer pathway. The successful applicant will be an integral part of this team working to put the patient at the centre of this rapid diagnostic pathway

Principle Activities

The post holder’s principal duties are based in Respiratory Medicine at the John Radcliffe hospital where the interventional and diagnostic lists occur, outpatient clinics which are currently located on the Churchill Hospital site and the inpatient ward is based at the John Radcliffe. It is a condition of the appointment that the postholder will be willing to work in any of the Trust’s locations. The post holder will be part of daytime and night-time respiratory on call provision. The rota operates on a 1 in 8 with residential day time cover until 9.00pm or 10.00pm and then on-call overnight. There will be an infrequent commitment to the hospital at night ward cover rota (approximately 2 weeks in 12 months)

The main role of the post holder will be to provide consistent support to the Respiratory Early Diagnostic Service team and Lung cancer and pulmonary nodule services as required under the supervision of the Lung Cancer consultants Dr Alastair Moore (Lung Cancer & Bronchoscopy Lead), Dr Ambika Talwar and Dr John Park (Departmental Lead for Respiratory Medicine)

Specifically, such duties may include:

Bronchoscopy and EBUS lists (including GA and interventional lists)

Lung Cancer Clinics

‘Virtual’ nodule follow-up clinics

Weekly Lung Cancer MDT

Attendance at nodule MDTs

Contribution to national and regional databases

Review/action test results

Involvement in audit, including National Lung Cancer Audit

Pleural Clinics

Pleural Intervention lists

Supervised training will be provided to the post holder as required. Training requirements will be reviewed at start and mid-way though post. It is envisaged that the post-holder will gain significant clinical experience in all aspects of Lung Cancer and pulmonary nodule management. It will be necessary for the post holder to work closely with the rotational specialist registrar (or equivalent) flexibly to ensure adequate middle grade cover

Teaching/Research

Oxford University Hospitals NHS Foundation Trust is a teaching hospital Trust, and the post-holder will be required to participate in programmes for teaching clinical students, training junior doctors and in clinical examinations. It is expected that this will be an integral part of every day clinical activity. The interventional team have recently purchased a state-of-the-art bronchoscopy and EBUS simulator and the post holder will be integral in developing a learning suite and simulation courses with this

Clinical Governance

The post-holder will participate in all clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trusts, and external accrediting bodies

Personal and Professional Development

The post-holder will be required to keep himself/herself fully up to date with their relevant area of practice and to be able to demonstrate this to the satisfaction of the Trusts. Professional or study leave will be granted at the discretion of the Trusts, in line with the prevailing Terms and Conditions of Service, to support appropriate study, postgraduate training activities, relevant CME courses and other appropriate personal development needs

Management

The post-holder will be required to work within the Trusts’ management policies and procedures, both statutory and internal, accepting that the resources available to the Trusts are finite and that all changes in clinical practice or workload, or developments requiring additional resources must have prior agreement with the Trusts. He/she will undertake the administrative duties associated with the care of his /her patients and the running of his/her clinical department under the direction of the lead clinician and/or directorate chair
