We have a great opportunity to join our supportive Community Team as an Assistant Care Manager. To be successful in this role you will need to use your initiative and demonstrate personal resilience as you could have exposure to challenging behaviours from people who access or need the service. An important part of the role includes carrying a case load and completing Care Act assessments in line with The Care Act 2014, and Mental Capacity Assessments, in line with The Mental Capacity Act 2005. You will be required to produce accurate, and evidence-based recording, and be able to explain Adult Social Care procedures for a variety of audiences including multi-disciplinary colleagues, families and people receiving a services.

As with all our teams, partnership and collaborative working is a vital aspect of decision making for implementing support. This role requires excellent communication and collaboration with partner agencies including, but not limited to, Hospital Social Work and Discharge Teams, Occupational Therapy (OT), The Safeguarding Hub, Support Brokers, Sussex Police, Health Services and Mental health services.

The role is exciting and busy however, you would be very well supported with a robust induction, and ongoing support from the team including the qualified social workers in the teams, you will always be learning, and no two days are the same. You would also have the option to work on a hybrid basis.

In the Adur community team you will be working across Shoreham and the surrounding areas, including up to the outskirts of Brighton and Hove. There are approximately 20 members in each of the Adur Community Social Work Teams, with a mixture of qualified and unqualified team members, so whether it's in person, on the phone or even in the chat group there is always someone on hand to offer help and support.

Experience and Skills

Key Skills:

  1. Ability to analyse and interpret information and prioritise needs in order to make recommendations/decisions which will have a positive outcome.
  2. Ability to work independently and as part of a team, working under pressure and to tight deadlines.
  3. Ability to work in a multi-agency environment to deliver a service to those people who access or need the service.
  4. Ability to communicate complicated or sensitive information to a range of audiences to achieve positive outcomes; to be able to communicate clearly with customers reflecting the service objectives and standards.
  5. Sound and accurate IT and keyboard skills.
  6. Ability to recognise the limits of own accountability and responsibility, and to seek appropriate support, advice and supervision.

Qualifications and/or experience:

  • GCSE English and Maths or equivalent qualification or experience operating at this level of literacy and numeracy.
  • NVQ Level 3 in Care. Desirable
  • Experience of providing support/working with people in a Social Care environment.
  • Experience of forming effective working relationships within a multi-agency setting.
  • An awareness of the policies, practices and procedures relevant to Social Work. Experience of the assessment and review process of care packages.
  • Experience of applying relevant legislation, policies and procedures to individual pieces of work.
