Summary
Key Responsibilities Deliver basic clinical care and health promotion interventions as delegated by registered staff, including basic observations, phlebotomy, nutrition. Monitor vital signs and report changes in condition to the supervising clinician. Assist in the implementation of care plans and contribute to their review. Promote self-care and independence, encouraging patients to engage in their own health management. Maintain accurate and timely documentation in line with GDPR and clinical governance standards. Participate in multidisciplinary team meetings and daily handovers. Escalate safeguarding concerns or deterioration in patient condition appropriately. Travel to patient homes across the PCN footprint (mileage allowance provided).
