Summary
To work as a team with other paediatric dietitians to provide dietetic care to diabetic out patients referred by the Child Health Directorate. To provide cover for other paediatric dietitians work during absence. To be responsible for the provision of an outpatient service for patients attending consultant led paediatric diabetes clinics. To attend case meetings as appropriate. To undertake appropriate nutritional assessment and dietary treatment of patients referred for dietary advice and design care plans. To review patients and adapt care plans as nutritional requirements change with age, stage of development and clinical status. To communicate with and educate patients and their parents/carers effectively to translate complex theory into a realistic, understandable format. To liaise with parents/carers, community health, education and social services in providing for the nutritional needs of patients in the community. To participate in the production and review of resources to ensure that they are evidence based, up-to-date and in line with Trust policies on document control
