Summary
Provide holistic assessment and evidence-based nursing care to stroke patients to enable them to achieve their best potential. To demonstrate stroke knowledge to educate patients regarding healthier lifestyles, blood pressure management and psychological support. Supervising/supporting the Band 3 Rehabilitation Assistants and working within an interdisciplinary team. Contribute to the development and evaluation of the nursing service To act as a source of expertise on stroke patients management. Liaising with GPs, Stroke Consultants and Third Parties. To maintain accurate comprehensive and contemporaneous documentation/patient records in line with legal requirements and Trust guidelines including personally generated clinical observations requiring computer skills.
