Summary

Responsible for the assessment, planning, implementation and evaluation of patients care within the Ward/unit, working in accordance with the philosophy of patient centred care and to NSF and other Trust standards. Carry out skilled, evidence- based nursing care. Demonstrate an analytical approach to clinical assessment of patients in the light of current research and resources. Work as a named nurse for a caseload of patients, coordinating the multidisciplinary assessment, planning, delivery and evaluation of care and treatment, ensuring patients have an individualised care plan. Evaluate and reassess nursing care according to clinical need. Work closely with the care coordinator, social worker and all Primary Health Care team members and others concerned, to facilitate safe discharge or transfer of patient care. Supervise care given by staff nurses and HCAs, Rehabilitation and other support members of the team. Delegate nursing care appropriately to junior staff. Deputise for / represent the Ward Manager / Matron/Hospital Manager in their absence and on a rotational basis hold the hospital bleep if necessary.
