Summary

Key Responsibilities The ANP will: 1. The post holder is expected to hold a qualification in Advanced Clinical Assessment skills and Non-Medical Prescribing (V300) with the ability to practice a high standard of professional conduct in line with the policies, protocols and guidelines of the Trust and NMC and Standard documents, ensuring that other members of the nursing team are aware of them. 2. Developing quality initiatives in standard setting and auditing programmes of care. Participating in research projects as required. 3. Work with clinical teams across CHN providing clinical care to a wide breadth of patients in line with their scope of professional practice which will continue to expand. Main Duties and Responsibilities Clinical Take a leading role in the enhanced management of patients who require enhanced care/treatment to prevent a hospital attendance. See patients in their own home/usual care environment as an autonomous practitioner (within scope of practice). Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undiagnosed and undifferentiated illness (within scope of practice). Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly (within scope of practice). Proactively identify, diagnose and manage treatment plans for patients at risk of deterioration outside of the hospital setting. Facilitate and support multidisciplinary team working with internal and external colleagues to aid clinical decision-making and promote patient safety. Refer onto specialist services if necessary Liaise with diagnostic services if patient requires further investigations. MANAGEMENT Support staff development in order to maximise potential. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Critically evaluate and review innovations and developments that are relevant to the area of work. Enlist support and influence stakeholders and decision-makers in order to bring about new developments in the provision of services. Lead clinical teams and ensure support structures are in place for the smooth running of relevant services.
