About the role:

Linkage Community Trust is looking for an experienced Senior Support Worker to join our team of Carers and Support workers as we develop a new and exciting service in and around the Gainsborough/Lincoln area.

This new role is an opportunity to join our care and support team as we continue to invest in the development of services within Lincolnshire and specifically this new location of Gainsborough.

We are looking forward to providing more services for individuals with a wider range of support needs across a wider area.

- You will have access to a Linkage vehicle to enable you to attend visits, meetings and carry out initial assessments as determined by the commissioners.

Who are You?

- You are an established professional working as a senior carer ideally in a Learning disability setting with good knowledge of undertaking assessments, creating care plans and identifying risks to ensure that individuals independence is promoted and managed in a positive way - as an existing Senior Carer or Deputy manager this is an exciting prospect to build upon existing knowledge whilst building new skills

- You are passionate about quality and committed to providing high quality care

- You are supportive of choice, enthusiastic and committed to championing the independence of the people we support

- You are able to remain calm in challenging situations with a considerate and understanding approach towards to needs of the individuals with learning disabilities and/or autism

About the role:

- You will work as part of the support worker team working across the Gainsborough area, providing short term care and support to people with a Learning disability and/or Autism

- You will facilitate discharges from hospital to residential settings or to the individuals own home, and/or prevent in patient admissions where possible

- You will contribute to and undertake initial assessments for individuals ensuring that person centred support plans are implemented and any identified risks are managed positively to promote independence

- You will support the service in providing a swift response to service requests and to act as the co-ordinator between the person needing the support and other organisations and agencies as needed.

- You will be flexible to requests for assessments and support, providing engagement at times that may be outside of core hours

- You will have experience of working with service commissioners, acting in a responsive manner upon receipt of referrals, and working flexibly to meet the needs of the individuals accessing the service

- You will be confident in maintaining accurate and detailed electronic records and have good IT skills
