Summary


Liverpool Diabetes Partnership are working with the GP practices of Liverpool we are offering an opportunity for a Health Care Assistant to work with the LDP team to support the consultants/nurses in the Liverpool area to care for their patients who live with Diabetes.

The successful candidates will assist the team in providing the highest possible standard of care for patients living with Diabetes, working in the community clinics in a clinical setting and in patients homes.

You will be working across the city within local communities, in a number of setting. It is essential that the successful candidate has a good understanding and knowledge of confidentiality

Under the supervision of registered clinicians, the HCAs will assist the team in providing the highest possible standard of care for patients and their families/carers. The post holder will undertake a range of tasks and duties as delegated by clinicians in a number of settings.

Participate in all aspects of patient care.

Engage with staff in General Practice to work collaboratively with their practice populations.

At the discretion of a Registered Clinician participate fully in the delivery of planned care.

Provide information to enable effectiveness of care to be evaluated and records maintained.

Involve patients and carers/relatives in the delivery of care

Provide reassurance, support to patients, family and carers

Work collaboratively with other professionals and agencies to ensure patient needs are met.

Establish and maintain effective communication with patients and carers/relatives

Recognise and respond appropriately to urgent and emergency situations

General observations such as blood pressure, pulse, urinalysis

Setting up clinic equipment

Undertake venepuncture as directed by the RN

Administrative

Provide and receive information verbally, in writing or electronically to inform work colleagues, patients, clients, carers, the public/external contacts. Dealing with anxious patients and families. Encounters language and cultural challenges

Demonstrate a competent level of inter-personal skills when providing information of a non-clinical nature to staff, clients, carers, public/external agencies, maintaining the Trust standards of customer service.

Deal with incoming/outgoing mail in accordance with the workplace procedures

Accurate recording of information
