Summary
Lead certain community services i.e. monitoring blood pressure and diabetes risk for elderly patients. Perform urgent and same-day home visits. This role will work as part of the PCN MDT, working collaboratively with primary care network staff to promote integrated and seamless pathways of care. Instigate necessary investigation/non-invasive diagnostic tests/investigations and interpret findings/reports. Refer patients to an alternative care setting or treat and discharge as appropriate. Contribute to the locality practices achieving its quality and quantitative targets to sustain the high standards of patient care and service delivery. Help to develop and set up new patient services and participate in initiatives to improve existing patient services.
