About the Role
We are currently recruiting on Care Coordinator, you will be responsible for clinical oversight and care governance across 10 services, supporting the operational and quality teams in delivering safe, person-centred care.
This is a pivotal leadership role for a registered nurse with a strong background in community-based nursing or complex adult care who thrives in a fast-paced, evolving environment.
Key Responsibilities
* Provide clinical leadership across 10 supported living services.
* Review and implement care plans for individuals with complex and high-risk needs.
* Act as the lead clinical liaison for referrals, transitions, and multidisciplinary team meetings.
* Support service managers in embedding best practice and compliance with CQC standards.
* Monitor incident reports, safeguarding alerts, and health outcomes.
* Lead on health audits, clinical risk assessments, and medication reviews.
* Train and mentor support staff in delivering quality, trauma-informed care.
* Work collaboratively with local authorities, NHS commissioners, and families.
Essential Requirements
* NMC Registered Nurse (RMN, RNLD, RGN with community or complex needs experience).
* Minimum 3 years’ post-registration experience in a clinical or care-coordination role.
* Previous experience in supported living, residential care, or community adult health.
* Strong understanding of CQC, DoLS, MCA, and risk management frameworks.
* Exceptional communication, leadership, and organisational skills.
* Ability to travel between services (mileage reimbursed).
Desirable
* Experience working with forensic clients or dual diagnosis.
* Experience supporting services through CQC inspections.
