Job Description: Prescribing Advanced Clinical Practitioner (ACP) We are seeking a qualified and experienced Prescribing Advanced Clinical Practitioner (ACP) to join our multidisciplinary team at Bridge Medical Centre. The post-holder will work autonomously within the scope of their professional competence, including independent prescribing, to deliver high-quality care to our patient population. Main Clinical Responsibilities Diagnose and treat patients presenting with minor illness, including safe and independent prescribing. Triage and manage patients requesting to see a healthcare professional, referring to other members of the primary care team where appropriate. Discuss patients requiring referral to secondary care with the registered GP prior to referral. Deliver safe and effective clinical practice within professional boundaries and recognised limitations. Advise patients on general health care and minor ailments, referring as necessary. Undertake home visits for acute conditions as assessed by the duty doctor and within the scope of qualifications. Ensure the delivery of evidence-based care to the highest standards. Communicate effectively at all levels within the team to support safe, coordinated care. Maintain accurate, contemporaneous healthcare records appropriate to the consultation. Conduct medication reviews and provide counselling on prescribed therapies. Prescribe medication for acute and chronic conditions in accordance with current guidelines and legislation. Professional and Administrative Responsibilities Ensure clinical systems (System One) are kept up to date with accurate documentation. Complete all necessary documentation associated with patient care and registration. Collect and maintain statistical information for audits and reporting. Attend and participate in team and practice meetings as required. Actively engage in the audit process and quality improvement initiatives. Participate in the administrative duties required as part of the practice team. Training and Personal Development Maintain continued professional development in accordance with revalidation and professional requirements. Undertake annual appraisal and participate in clinical supervision where appropriate. Participate in the training and induction of staff and students. Attend relevant courses and study days for professional development. Take responsibility for own learning, development, and performance. Pathological Specimens and Investigatory Procedures Collect pathological specimens, including blood samples and swabs, as necessary. Perform investigatory procedures requested by GPs. Safeguarding and Governance Recognise and act upon safeguarding concerns in line with local and national guidance. Remain accountable for prescribing decisions and clinical judgement. Ensure all care provided complies with relevant professional and clinical governance standards. Essential Qualifications and Experience Registered Nurse or Paramedic with current NMC/HCPC registration. MSc in Advanced Clinical Practice (or equivalent). Independent Prescriber (V300 qualification). Demonstrable experience working autonomously in a primary care or equivalent setting. Desirable Experience in a GP surgery. Familiarity with System One.
