Summary
Applicants should note that it is a condition of employment under the Additional Roles Reimbursement scheme, that the Pharmacy Technician is registered with the General Pharmaceutical Council (GPhC). Furthermore, if not already qualified from or undertaking the Primary Care Pharmacy Educational pathway, the successful candidate will be enrolled and must complete an approved training course. Medication Management To undertake face to face, telephone and email consultations with patients Once trained and competent, to take responsibility for managing at least one clinical area (such as medication reviews, DOAC reviews, etc) To be actively involved with QOF domains, PQS domains, CQC safety searches and any other relevant safety administration To undertake monitoring of high risk medicines DMARDs, etc To action Care Home monthly repeat medication requests To undertake medication reviews To provide advice and information to support the improvement of prescribing safety, quality, cost-effectiveness, and the appropriate use of medicines to get the best possible patient outcomes To assist in monitoring and management of prescribing budgets and the preparation of evidence-based resources and information that support cost-effective prescribing To provide expertise to address the public health and social care needs of patients, including lifestyle advice, service information and help in tacking health inequalities To carry out medicines optimization tasks including checking inhaler technique, etc To manage Shared Care Agreements/Protocols Ensure ICB prescribing points have been sent out Contact community pharmacy for dosette and patient queries Medicines Reconciliation To review secondary care requests for new medication (as communicated by discharge summaries, outpatient letters, etc.) and raise any queries with the Practice Pharmacist. To put these requests on the patients record To put repeat medications for new patients, who have just registered with the practice, onto the record (from repeat prescription printout) and suggest GP medication review where appropriate. Low threshold to run by Practice Pharmacist Change creams and ointments as per formulary choice Action all discharge summaries and clinic letters received via Docman To maintain cost-effective prescribing as per BOB Formulary Prescribing Systems and Policies To work with the Practice Pharmacist on any new prescribing policies To complete end of month submissions to NHSBSA and act on any potential payment discrepancies Liaison with community and hospital pharmacies: To support and advise reception when needed. Reception will deal with most community pharmacy queries initially and pass those they cannot manage onto the Practice Pharmacy Technician To check any community pharmacy medicine use reviews (MURs) to see if action needed and pass to the relevant clinician To promote NMS To continue developing good working relationships with local community pharmacies
